How do I set up an appointment?

You can Book a Scheduled Appointment online here, or give us a call (727) 201-2538. We accommodate patients with convenient appointment times that fit their schedule.

Do I need to bring documentation?

For some instances you may need to bring documentation with you to better serve you with your appointment. You can always request medical records if you do not already have them available. You can sign an authorization with who carries your records. Staff can try to assist you with obtaining the right documents.

Type of Payments Accepted?

Debit or credit cards such as Discover, American Express, MasterCard and Visa. As well as cash and checks.

Accept Health Insurance?

For certain appointments, there may be availability of Insurance usage. Whether medicaid, medicare, or any other form may be accepted.

How long are Appointments?

Appointment times may vary depending on what service is being booked.

Privacy Concerns?

Privacy for patients is always important to us. There are privacy laws such as the Health Insurance Portability and Accountability Act which help in protecting your Doctor Visit.

Refund Policy?

After successfully receiving service from your scheduled appointment, there are no refunds available, and all outstanding payments are due and obligated to be paid. If payment is collected monthly the remaining invoice(s) and billing cycle(s) are obligated to be paid automatically per your agreement. Otherwise, our refund policy is as follows:

Customers with Monthly Payments:
– Same Day cancellation, upon refund request, receive complete 100% refund.
– Cancellation After 24 Hours, upon refund request, receive refund of the latest charge.

Customers with One-Time Payments:
– Same Day cancellation, upon refund request, receive complete 100% refund.
– Cancellation After 24 Hours, and before receiving service from attending your scheduled Doctor visit, a cancellation fee may apply, not to exceed $20, deducted from your refund.

Billing Discrepancies

Each customer agrees to provide LMC with atleast 30 days to attempt settlement of any billing discrepancies before initiating disputes with any third-party credit/debit card company or bank. Customer agrees and consents completely for any and all disputes, sole jurisdiction shall be in maintained in court located only in Pinellas County, Florida.

Allow Rescheduling Appointments?

You can for most appointments. Anything regarding appointment changes, clients need to contact us right away. Yes, you can reschedule if you provide an early notification. Please understand our Doctors are hard workers and prepare ahead to provide a quality experience. There may be a fee of $30 for all appointment no-shows, unless our staff is notified atleast three (3) business days before the scheduled appointment. For late cancels, a fee may apply, not to exceed $20, deducted from your refund.

Total Amount Requested Different with my provider?

Some Doctor Offices provide their own custom levels of quality service, and their availability for scheduling is all unique. Therefore not all Doctor Office appointments may be alike and can require a different amount for their own service, which can have costs higher than the typical lowest price offered by other Doctors on our network. All customers are guaranteed to receive high-quality service with all of our certified Doctors. Maybe your on a budget or the price is an issue. You can go check if we have other Offices in your area with cost and plan structure to better fit your needs.
